How it works
Three steps from referral to follow-up.

You refer a patient
A physician, discharge planner, or facility administrator sends us a patient or resident who needs coverage or member assistance.

We handle enrollment
Medi-Cal, Medi-Cal managed care plans, Medicare, Advantage, Supplement, and Part D. We help the patient choose and enroll.

A Community Health Worker follows up
Someone from the same community helps the patient stay healthy and get the care they need after enrollment.

Community Health Workers
A Community Health Worker is someone who helps people stay healthy and get the care they need. They often come from the same community as the people they help.
Medi-Cal Enrollment
Free or low-cost health coverage to people who qualify. Benefits include Doctor Care, Emergency Care, Medications, Dental & Vision, Lab Tests, Hospital Care and more.
Medicare Enrollment
Catering to the needs of senior citizens and permanently disabled individuals to help them navigate Medicare plan options and benefits in the effort to help maintain and improve quality of health and lifestyle.
Community Outreach
Engaging with the community to offer resourceful information, programs and services which will be instrumental in improving quality of life and health.
Supplemental Plans
Supplement plans help cover your portion of cost-sharing with Medicare, including deductibles, copayments, and coinsurance.
Prescription Drug Plans
You do not enroll in Medicare Part D via Social Security. Instead, we help you choose a Medicare Part D plan offered by a private insurance company in your state.
